Important information about the patient is provided by the so called “Pflegeüberleitungsbogen”, a document that is created by the hospital or the treating doctor and contains relevant details for transferring the patient. This document enables us and the family to begin or resume care in the home and family environment.

On the other hand we also create a transfer sheet at the beginning of the care that always gets updated to make a sometimes inevitable hospital stay much easier for the patients, families, doctors and nurses.
